Contest Rules and Regulations
ELIGIBILITY: No purchase necessary. Contest is open to all legal residents of Canada (excluding Quebec) who have reached the age of majority in their province/territory/state of residence at the time of entry, except employees or agents (and those with whom they are domiciled) of (collectively, “The Contest Parties”):
a. MD Beauty Clinic Inc. (the “Sponsor”);
b. The Plastic Surgery Clinic Inc.;
c. Miracle 10 Cosmetics Inc. ;
d. any and all advertising and promotion agencies retained by or associated with MD Beauty Clinic Inc., The Plastic Surgery Clinic Inc. and Miracle 10 Cosmetics Inc.; and
e. the Contest Judges.
HOW TO ENTER:
During the Contest Period (starting October 21st, 2024 and ending at 11:59 November 16th, 2024) the contestant must successfully fill out the contest ballot (hereinafter the “Contest Ballot”). There is 1 limit of entries per contestant. Use of any automated system to enter the Contest is prohibited. Contestants who use an automated system to enter the Contest or who submit more than one entry will be automatically disqualified from the Contest and all submitted entries shall automatically be deemed to be invalid. No Contest entry is valid unless a) it is made through the submission of a Contest Ballot; b) the entry contains all information required by Contest Ballot; and c) the Contest Ballot is submitted and received during the Contest Period.
THE PRIZE(S) AND APPROXIMATE RETAIL VALUE:
The Prizes shall consist of one (1) of two (2) Full Face Rejuvenation Treatments (valued at $2,500). Following an initial consultation, your aesthetician will determine a combination of treatments and skincare products that amount to the value of the prize. This may include a package of treatments. Alternative treatments may be suggested by MD Beauty Clinic at our sole discretion. The winner must undergo an in-person consultation with an aesthetician or medical nurse before undergoing any treatment for the first time. There is one (1) prize available to be won for each randomly selected winner. The Prize must be accepted as awarded and is not transferable or convertible. No substitutions will be made except at the Sponsor’s option. The prize will only be awarded to the person whose name appears on the Contest Ballot.
THE PRIZE WINNER SELECTION:
On November 16th, 2024 (the “Prize Draw Date”) in Toronto, Ontario at approximately 12:00 p.m. ET. two (2) Qualified Eligible Entries will be selected by random draw from among all Qualified Eligible Entries received during the Contest Period (the “Prize Selected Entrant”). The Sponsor will make three (3) attempts to contact the Prize Selected Entrant between November 16th, 2024, and November 24th, 2024 (the “Prize Notification Period”). In the event that the Sponsor is unsuccessful in contacting the Prize Selected Entrant within the Prize Notification Period, then the Prize Selected Entrant will be disqualified and an alternate eligible entrant will be randomly selected from amongst the remaining eligible entries.
GENERAL CONDITIONS:
Before the Prize is awarded, the Selected Entrant will be required to:
a. successfully answer a skill-testing question; and
b. agree to have treatment filmed/photographed for marketing use; and
c. read, and agree to the Sponsor’s Contest Rules and Regulations;
All entries become the property of the Sponsor, which assumes no responsibility for lost, delayed, incomplete, illegible and/or misdirected entries. This Contest is subject to all applicable federal, provincial and municipal laws. The decisions of the Contest judges with respect to all aspects of this Contest are final and binding on all entrants without right of appeal, including, without limitation, any decisions regarding the eligibility/disqualification of entries. The Sponsor reserves the right to cancel or suspend this Contest, or to amend these Rules without prior notice or obligation, in the event of any accident, printing, administrative, or other error of any kind.
NO WARRANTY OR REPRESENTATION: ALL CONTEST ENTRANTS HEREBY EXPRESSLY ACKNOWLEDGE AND AGREE THAT, IN CONDUCTING THE CONTEST, NEITHER THE SPONSOR NOR THE CONTEST PARTIES MAKE ANY WARRANTY OR REPRESENTATION WHATSOEVER, WHETHER EXPRESS OR IMPLIED, AS TO THE SUITABILITY FOR USE OF CoolSculpting OR ANY NON-SURGICAL TREATMENTS BY THE PLASTIC SURGERY SKINCARE CLINIC AND THE PLASTIC SURGERY CLINIC WITH RESPECT TO ANY PERSON(S), INCLUDING THE PRIZE SELECTED ENTRANT, OR THE EFFECTIVENESS OF SUCH TREATMENT.
COLLECTION OF PERSONAL INFORMATION:
By entering this Contest, each entrant expressly consents to the Sponsor, its agents and/or representatives, storing, sharing and using the personal information submitted with his/her entry only for the purpose of administering the Contest and in accordance with Sponsor’s Privacy Policy unless the entrant otherwise agrees. You choose to allow this information to be used by the Sponsor and other Contest Parties for promotional purposes (which purposes may include email newsletters and other correspondence containing product information and/or promotions) by selecting the option upon submission of your Contest Ballot. If you choose to do so but later change your mind, you can opt-out at any time.
